If you are interested in coming for treatment at Carlisle House we need to find out a bit about you. You can either give us a call on 028 9032 8308 and we'll send you out an application form - or - you can fill out the application form online.
The online application form will take maybe 30 minutes to complete.
What happen's after I submit the application form? Your form will be reviewed by our staff and if selected you will be asked to come for an interview. At the interview you will be required to sign a copy of the submitted application form which we will have printed off.
